Important Reminder:  A doctor's referral is required before visiting the clinic.  Let your doctor know that you will need this document in order to be seen by a Wound and Ostomy Care Nurse (WOCN) and that there may be other instances that will require another visit with the Nurse.

Request that they either give you the referral form or fax it to the clinic you will be visiting.
